FDA Medical Devices Safety Alerts and Notices – updated October 6, 2014

Consider inspecting the inside of the devices (arthroscopic hand pieces) following cleaning to ensure that they have been cleared of any tissue or fluids. There may be multiple ways to accomplish this. As one example, the facility that brought this situation to our attention uses a 3mm video scope to inspect the channels of the shaver hand piece.

• The INSPEKTOR™ is 2 mm and flexible.  It works great inspecting arthroscopic shaver hand pieces

FDA Website - Workshop on Medical Device Cleanliness: How Clean is Clean Enough? November 10, 2010

An endoscope was used to inspect 494 suction tubes that had gone through a large teaching institution’s surgical instrument processes. Researchers found foreign matter in every suction tube (100%).

• The INSPEKTOR™ offers a quick and easy way to visually  inspect instrument lumens

FDA Patient Safety News – Cleaning Procedures for Arthroscopic Shavers.  September 2009

…facilities that use arthroscopic shavers should evaluate the adequacy of their cleaning procedures, and should ensure that the personnel who clean and sterilize these devices follow all the steps in the manufacturer's cleaning instructions prior to sterilization. They should also consider inspecting the inside of the devices after cleaning to ensure that they have been cleared of any tissue or fluids.

• The INSPEKTOR™ is 2mm and flexible.  It works great inspecting arthroscopic shaver hand pieces and most lumen instruments.

ANSI/AAMI ST79 – 7.5.5 Verification of the cleaning process

After the cleaning process, personnel should visually inspect each item carefully to detect any visible soil.  Inspection using magnification might identify residues more readily than the unaided eye.

• The INSPEKTOR’s  9X magnification is great for inspecting instrument’s lumens and exterior hard to see places

APIC Bulletin –  Investigators discovered that infections were caused by the use of dirty surgical instruments

Investigators discovered that infections were caused by the use of dirty surgical instruments (arthroscopic shavers, which surgeons use to shave away bone and tissue during surgery; and cannulas, long narrow metal tubes used to irrigate and suction the surgical site). The result of this investigation, led by Tosh and colleagues, was published in an article titled “Outbreak of Pseudomonas aeruginosa Surgical Site Infections after Arthroscopic Procedures: Texas, 2009,” in the December 2011 issue of Infection Control and Hospital Epidemiology.

• The INSPEKTOR™ is designed to visually inspect instrument’s hard to see or hidden areas at 9X magnification.  It is fast and easy to use.

Burden - US

➢ ~300,000 SSIs/yr (17% of all HAI; second to UTI)

➢ 2% - 5% of patients undergoing inpatient surgery

Mortality

➢ 3% mortality

➢ 75% of deaths among SSI patients are directly attributable to SSI

Morbidity

➢ Long-term disabilities

➢ 7 - 10 additional postoperative hospital days

Cost

➢ $3000 - $29,000/SSI depending on procedure & pathogen

➢ Up to $10 billion annually

➢ Researchers tabulated the annual SSI cost at $3.3 Billion

5. Strategies for the Prevention of Surgical Site Infections Review of New Multi-specialty Society Guidelines, August 15, 2012

Researchers reviewed data from 1986 to 2013 on costs associated with individual HAIs and found the following costs on a per-case basis: Surgical site infections —

$20,785.00

Researchers tabulated the annual surgical site infection (SSI) cost at $3.3 billion.
